What is Form W-12?

Form W-12 is the IRS PTIN Renewal Application. Every paid tax return preparer must have a valid PTIN and renew it annually — typically between October and December for the following tax year. The fee is $19.75 per year.

Without a valid PTIN, you cannot legally prepare tax returns for compensation. Your PTIN is essentially your license to practice.

What Does Line 11 Say?

Line 11 of Form W-12 asks you to certify your compliance with data security requirements. Specifically, it asks whether you have implemented a Written Information Security Plan as required by IRS Publication 4557 and the FTC Safeguards Rule.

This certification is made under penalty of perjury — the same legal standard as signing a tax return.

What Are Your Options?

When you reach Line 11, you have three realistic options:

Option 1 — Certify truthfully that you have a WISP

This is the correct answer. You have a documented Written Information Security Plan on file, you've implemented its requirements, and you review it annually.

Option 2 — Certify that you don't have one

This is honest but creates problems. The IRS may flag your renewal, delay processing, or require you to correct the deficiency before issuing your PTIN.

Option 3 — Falsely certify that you have one when you don't

This is the worst option. It exposes you to federal perjury charges, PTIN revocation, and potential civil and criminal liability.
